Skin Maintenance Membership

This agreement outlines the terms and conditions of your Skin Maintenance Membership with TKO Beauty Tech.

The Skin Maintenance Membership is designed to support skin health, preserve results, and maintain consistency through ongoing professional care. This membership focuses on maintenance, not corrective or intensive treatment.

Membership services are intended for clients who have completed a corrective program or whose skin is currently stable. Clients who require active skin repair, tightening, or corrective protocols may be recommended a separate treatment plan following a consultation.

Each plan includes:

One maintenance-focused facial treatment per month
Priority booking
Monthly skin-care guidance and maintenance support resources

SCHEDULING & NO-SHOW POLICY

A. Appointment Scheduling It is the member's responsibility to schedule their monthly facial in advance. Appointments are subject to availability, and TKO cannot guarantee specific time slots if bookings are delayed.

B. Missed Appointments / No-Shows If you cancel or reschedule less than 48 hours before your appointment, or fail to attend, it will count as a missed session for the month and cannot be rescheduled.

Maintenance Scope Acknowledgment *

I understand this membership is maintenance-focused and does not include corrective or intensive treatments.

Recurring Billing Authorization *

I authorize TKO Beauty Tech to securely store my payment method and automatically charge my card on the 1st of each month for my selected membership and any applicable add-ons.

Early Cancellation Acknowledgment *

If I cancel before the end of my commitment, I agree to be charged one final monthly fee (equal to my current membership rate), which covers services and benefits already provided under the membership.
