MEDICAL AUTHORIZATION AND LIABILITY RELEASE
I authorize Bling Athletics LLC (“Bling Athletics”) and its authorized representatives to obtain or consent to necessary medical treatment for my child if I cannot be reached. I understand that participation in cheerleading, dance, tumbling, stunting, conditioning, competitions, performances, camps, clinics, private lessons, and other athletic activities involves inherent risks, including the possibility of serious injury.
I knowingly and voluntarily assume the risks associated with my child’s participation. To the fullest extent permitted by California law, I release, waive, discharge, and hold harmless Bling Athletics LLC, its owners, members, managers, employees, coaches, staff, volunteers, representatives, and agents from claims, demands, damages, or liability arising out of or related to my child’s participation in Bling Athletics activities, including claims resulting from the ordinary negligence of Bling Athletics or the released parties, except to the extent such release is prohibited by law.
I understand that Bling Athletics takes reasonable measures to provide a safe training environment but cannot eliminate all risks associated with athletic participation. My athlete and I agree to follow all safety rules, policies, and instructions provided by Bling Athletics.
PARENTAL CONSENT
I give permission for my child to participate in Bling Athletics programs and activities. I understand that my child must follow all rules, policies, and instructions established by Bling Athletics coaches and staff and that failure to do so may result in removal from a class, practice, event, team, or program. I acknowledge the inherent risks associated with athletic participation and voluntarily consent to my child’s participation.
PHOTO & MEDIA RELEASE
I grant Bling Athletics permission to photograph and/or record my child during Bling Athletics programs, practices, competitions, performances, and events and to use such images or recordings for legitimate promotional, advertising, website, social media, and other business purposes without compensation.
