About you

Use your full legal name, as it appears on your ID.
Used for he/she wording in your documents.

Your health care agent

Your agent is the person who makes health care decisions for you if you can't make them yourself.
Steps in if your agent can't or won't serve.
The California form starts your agent's authority only when your doctor decides you can't decide for yourself, unless you choose right away.

End-of-life choices

These two choices are copied from the California form. Choose the one that matches your wishes, or leave it to your agent.
The California form says comfort care and pain relief are always given, even if they may hasten death, unless you list exceptions.

Organ and tissue donation

Example: kidneys, corneas
Leave all unchecked to allow any of these.

Your doctor

You can also list a backup doctor.

Medical records access

After death

Burial or cremation, type of service, where, and where your cemetery plot is if you have one.

Anything else

Anything else you want your agent and doctors to know, such as religious beliefs or treatments you do or don't want.
Any existing advance health care directive or related documents.
