STATEMENT - I, the parent/guardian of the above-named child, hereby give my approval for their participation in activities during the current season. I assume all risks & hazards incidental to the conduct of the activities & transportation to & from activities. I do further release the Brantley County Rec Dept, the organizers of the activity, the sponsor, the supervisors, and any or all of them.

STATEMENT - In case of injury to my child, I hereby waive all claims against the organizers, the sponsors, or any of the supervisors appointed by them. I likewise release from responsibility any person transporting my child to or from the activity. I; the parent/guardian of the above-named child, have read & understand the concussion information page.

STATEMENT - I, the parent/guardian of the above-named child, understand that the Brantley County Parks and Rec Dept has permission to photograph my child for promotional & information purposes.
