John Barnard Uplifting Communities Award: Nomination Form

Nominees for the John Barnard Uplifting Communities Award must be:

Two (2) or more years of service in a community

Actively involved in promoting whole food plant-based nutrition and/or lifestyle medicine.

Demonstrate a history of work that positively impacts community health and well-being.

Focus on engaging and supporting underserved and under-resourced communities.

Work is under the umbrella of a 501c3 nonprofit organization

Winners must commit to attendance in-person at The Power of the Plate Conference presented by The Plantrician Project - Loews Coronado Bay Resort, California from May 14-17, 2026 (grant recipient will be awarded travel and tuition for the conference - learn more at www.poweroftheplate.org)

U.S.-based applicants only. Awards must be made to a nonprofit to advance community work.

Please note: While there will be one honoree annually, all submissions may be considered for Plantrician Project Community Story features to amplify important community-based work across the United States.
