Retreat Dates: October 2-5, 2025

Location: St George, UT

By signing below, I acknowledge and agree to the following terms as a participant in the Piper Blue Organic Makeup retreat:

1. Assumption of Risk:
   I understand and accept that participation in the retreat, including all related activities, may involve inherent risks such as physical injury, illness, emotional distress, and other potential hazards. I voluntarily assume all such risks.

2. Waiver and Release of Liability:
   I release and hold harmless Piper Blue Organic Makeup, its owners, affiliates, employees, volunteers, agents, and representatives from any and all liability, claims, demands, causes of action, or damages, including injury or loss, arising out of or relating to my participation in the retreat, whether caused by negligence or otherwise.

3. Indemnification:
   I agree to indemnify and hold harmless Piper Blue Organic Makeup from any claims, liabilities, damages, or expenses (including attorney’s fees) resulting from my conduct, actions, or participation during the retreat.

4. Medical Treatment Authorization:
   In the event of an emergency, I authorize Piper Blue Organic Makeup and its representatives to secure necessary medical treatment on my behalf. I agree to assume full financial responsibility for any medical services rendered.

5. Media Release:
   I grant Piper Blue Organic Makeup the irrevocable right to use any photographs, videos, or recordings taken of me during the retreat for marketing, promotional, and commercial purposes, without compensation or further approval.

6. Jurisdiction and Governing Law:
   This agreement shall be governed by and construed in accordance with the laws of the State of Utah. Any disputes arising under or in connection with this agreement shall be resolved exclusively in the courts of Washington County, Utah.

7. COVID-19 Release and Health Acknowledgment:
   I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending the retreat. I agree that Piper Blue Organic Makeup, its owners, affiliates, employees, volunteers, agents, and representatives are not responsible for any exposure to or contraction of COVID-19 or any variant thereof. I agree to follow all health guidelines and protocols established by retreat organizers and local/state health authorities. I confirm that I will not attend the retreat if I am experiencing symptoms of illness or have recently tested positive for COVID-19 within a period considered infectious by current health guidelines.

8. Acknowledgment of Understanding:
   I have read this Waiver and Release of Liability in full. I fully understand its terms and understand that I am giving up substantial rights by signing it. I sign it freely and voluntarily without any inducement.
