Payment plans are non-refundable
Payment Plan Timing Guidelines:
If purchased before your visit, the care plan will be applied to your next visit.
If you have just completed your first visit and would like your first visit to be covered by your package, the following conditions must be met:
The plan must be purchased by midnight on the date of your first visit.
If your first visit falls on the last day of the month, the plan must be purchased by 5:00 PM that day.
If the plan is purchased after the date of your first visit, it will be applied to all future visits only
Payment plans cannot be back applied to previous visits or products
Please note that a ~3% surcharge is included in the price of packages utilizing payment plans.
8 visit care plans expire after 9 months
12 visit care plans expire after 12 months
