I understand and acknowledge the risk that may expose me to certain foreseeable and unforeseeable risks of bodily injury, adverse effects, which may require me to receive medical attention or treatment. I knowingly and voluntarily assume all risks of the treatment by the licensed physical therapist, the use of the sports and physical fitness equipment at Rise Rehab & Performance, and/or on-site events and my participation in the programs/events of Rise Rehab & Performance. I hereby release Rise Rehab & Performance and their employees and agents from liability connected to my use of the sports and physical fitness equipment at Rise Rehab & Performance and my participation in any programs and events at these facilities or on-site events.

I give permission for my photo to be used for promotional and informational materials associated with Rise Rehab & Performance such as brochures, website and social media.

I have carefully read this Release of Liability and I have had the opportunity to ask questions about it. I fully understand its contents and knowingly and voluntarily agree to the terms and conditions set forth herein.

I agree to Rise Rehab & Performance to send me marketing emails at times.
