Biocanic Custom Branding Form
We use this to find your Biocanic account, so it's important it is the same as the email that the payment is under.
If someone besides the main contact above is handling the setup, please add that email here.  

Desired Biocanic portal URL

We will set up a custom Subdomain as part of your custom branding. If you have a website URL, we recommend using ________.YOURWEBURL.com, but you can also use COMPANYNAMEorACRONYM.biocanic.com.

For example: whb.biocanic.com or portal.myhealthclinic.com

If left empty, tab will be titled "Biocanic" on default.
Most used color for your practice/logo.
Second most used color for your practice/logo.
Only Google fonts are supported. Please choose a font from: fonts.google.com

Primary Brand Logo

We will need your brand logo with transparent background in a 4:1 ratio (e.g 400px wide x 100px tall).

Secondary Brand Logo (optional)

If you have a secondary brand logo (logo for light vs dark backgrounds), please upload the alternative logo here.

Brand Favicon

We will need your brand favicon with transparent background in a 1:1 ratio (e.g 100px wide x 100px tall).
