Board of Directors: List the name and address for each of your directors. You must have at least three directors. Although your state might allow for a lower number of Directors to serve, the IRS has a higher threshold. Please be sure to include yourself if you will be acting as a director.
Executive Directors
Officers: List the name (and address, if not already listed above as a director) for each of your Officers
List the address of your Registered Agent (must be in the state of your nonprofit and cannot be a PO Box)
List the address of your Principal Office (must be in the state of your nonprofit and cannot be a PO Box
Purpose Statement: Describe in one to two sentences the purpose of your corporation. Be sure to include why your corporation is being formed or what does your corporation intend to accomplish, who will benefit from the Corporation’s accomplishments, and how will the Corporation achieve its purpose:

Additional Activities: If your organization (i) will have any international or political involvement (such as attempting to influence legislation); or (ii) plans to donate funds to individuals or pay expenses of individuals (other than the reimbursement of expenses relating to your organization), please provide a description of these activities below.
