I have read and understand the Service Liability & Release form posted at inksquadstudio.com/service-liability. I adopt its statements for the service I am receiving today, I voluntarily accept the risks it describes, and I agree to its release of liability to the fullest extent permitted by Arizona law.

I confirm that I am signing voluntarily and sober, that the information I have given is true and complete, and that I am eighteen or older, or a minor receiving a piercing with my parent or legal guardian present as required by A.R.S. § 13-3721.
