All training recommendations from an EVALUATION expire after 30 days due to an increased chance of behavioral changes

When on-site or present for training, dogs must remain on leash at all times unless clients are otherwise instructed

Sick or contagious dogs are not permitted on-site or to attend any training sessions

MEDIA RELEASE: Owner consents to their dog being the subject of the photographs and/or audiovisual recording and authorize Wolfman’s Canines LLC to use them for educational or promotional purposes

This PRE-EVAL FORM shall be legally binding between the CLIENT and Wolfman’s Canines LLC. By signing below and attending or participating in any and all training programs, the CLIENT is agreeing to the conditions laid out in the above WOLFPACK POLICIES and has had a chance to express any and all concerns either verbally or in written form. The above agreement releases Wolfman's Canines LLC, its staff, owners and any representatives from any and all liability which the CLIENT or their dog(s) may suffer including but not limited to injury, sickness, damage, or death resulting from participation in training or boarding. Should the CLIENT assert a claim contrary to what has been agreed to in this PRE-EVAL FORM, the CLIENT and associated claiming party shall be liable for all expenses (including attorney’s fees, court costs and consultant fees) incurred by both the CLIENT and Wolfman’s Canines LLC. No waiver or modification of any provision herein shall be valid unless expressly agreed to in writing by both the CLIENT and Wolfman’s Canines LLC.
