Do you prefer whole bones, ground bones, or bone meal? *

Please note: Some recipes may require a combination of whole or ground bones AND bone meal but your preferences will be taken into account. If you choose ground bones, please note whether you have a grinder or if you will be purchasing your ground bones from somewhere. If you will be purchasing ground bones from somewhere please list what the ground bones are and where you are getting them from. Website links are preferred.

Please list all bone sources that are available to you and your preferences including the name of the bone source and the animal it's coming from. *

Ex: Chicken wings, chicken drumsticks, duck necks, turkey necks, pork ribs, chicken carcass. I have no preferences, I can source all of these easily. ***PLEASE LIST AS MANY OPTIONS AS POSSIBLE*** Note: If you ordered 1 meal plan - necks cannot be used. Dogs being fed exclusively necks as their bone source is dangerous and can permanently damage thyroid function.

Please list all organs available to you and your preferences including the name of the organ and the animal it's coming from. *

Ex: Beef liver, pork liver, chicken liver, beef kidney, beef spleen, pork kidney, chicken liver, chicken hearts. I prefer using beef & pork organs. ***PLEASE LIST AS MANY OPTIONS AS POSSIBLE***

To ensure your dog’s meal plan is nutritionally complete, we will include the following essential nutrients. Please confirm your preference for each; either using a supplement or allowing us to use a natural whole food source (if available).

Eggs are an excellent source of choline, biotin, and other essential nutrients. Are you okay with including eggs in your dog’s meal plan? If No, we will use supplements. *

Known Food Allergies/Intolerances *

Please be thorough. If any known intolerance, allergy, or sensitivity is not listed in this submission an adjustment fee will need to paid if any adjustments based on an unlisted intolerance, allergy, or sensitivity are required. If you have done an allergy/sensitivity test of some kind simply put "Test Sent To Email" and email your test results to [email protected].

Is there anything you will NOT feed your dog? *

Some people refuse to feed certain foods whether it be for personal preference, religious reasons, etc... Please list any foods you will not feed your dog below. If there isn't a particular item you won't feed just say "nothing".

Special Notes *

If there were any special requests or specific details that we discussed in a live chat, Facebook message, email, or other types of communication, please list the important details of that conversation here or reference the location of the conversation and the name I can find the conversation under. I.E. If your FB name is different than your real name. Please note that any specific requests for the meal plans must be included in your form submission. For example requesting a specific bone type or protein source and then changing your mind after the meal plans are delivered will result in an adjustment charge as these meals take a great deal of detail, time, and consideration.
