I certify that my dog is healthy enough to participate in training activities.

I understand that while every reasonable precaution is taken, working with animals carries inherent risks.

I authorize emergency veterinary care if necessary and accept financial responsibility for such care.

I understand payment policies, scheduling policies, and cancellation policies.

I agree to follow trainer instructions and recommendations during and after training.

CARE OF DOG  

Facility agrees to exercise reasonable care in the handling and boarding of Owner’s dog(s). Owner acknowledges that dogs are housed, exercised, and managed in an environment with inherent and unpredictable risks including but not limited to interaction with people, equipment, and other animals.

HEALTH & VACCINATION REPRESENTATION  

Owner certifies that each dog:   - Is current on required vaccinations   - Is free of contagious disease or parasites   - Has not exposed others to communicable illness within the past 30 days   - Owner agrees to provide accurate medical and behavioral information.

ASSUMPTION OF RISK  

Owner understands and accepts that despite reasonable care, risks include illness, injury, escape, stress, or death arising from normal kennel operations, transportation, or unforeseeable events. Owner assumes these risks on behalf of the dog.

RELEASE OF LIABILITY  

Except in cases of gross negligence or willful misconduct, Owner releases and holds harmless Facility, its owners, employees, and agents from claims arising from:   - Injury, illness, escape, or death   - Actions of the dog toward people, animals, or property   - Fire, theft, natural disaster, or unavoidable events  

INDEMNIFICATION  

Owner agrees to indemnify and defend Facility against any claims, damages, or expenses resulting from the behavior or condition of Owner’s dog(s), including bite incidents or property damage.

VETERINARY CARE AUTHORIZATION  

If Owner cannot be reached, Facility may obtain veterinary treatment deemed necessary.  Owner agrees to pay all veterinary expenses incurred.

PAYMENT TERMS & LIEN RIGHTS  

Owner agrees to pay all boarding and related charges.  Under South Carolina law, Facility retains a lien on boarded animals for unpaid expenses and may enforce this right as provided by statute.

FAILURE TO PICK UP / ABANDONMENT  

If Owner fails to retrieve the dog within the agreed time:   - Boarding charges continue to accrue   - Facility will attempt written notice   - After ten (10) days and as permitted by South Carolina law, the dog may be transferred or otherwise handled pursuant to statutory authority  Owner remains responsible for all costs.

BEHAVIOR & SAFETY  

Facility reserves the right to refuse or terminate services for dangerous or disruptive dogs. Owner remains responsible for all resulting costs.

PERSONAL PROPERTY

Facility is not responsible for loss or damage to collars, bedding, toys, or personal items left with the dog.

ENTIRE AGREEMENT

This agreement represents the entire understanding between parties and is governed by the laws of South Carolina
