Membership benefits are intended to encourage consistency in your health and wellness journey and may evolve as additional member offerings become available.

Membership Billing & Payment Authorization

By enrolling in the Misama Wellness Membership, I authorize Misama LLC to automatically charge the payment method on file according to the membership selected.

I understand and agree that:

Membership dues will be automatically charged each month.
Membership payments are non-refundable once processed.
I am responsible for maintaining a valid payment method on file.
If my payment method expires or a payment is declined, I agree to provide updated payment information within five (5) business days.
Failure to update my payment information within five (5) business days may result in cancellation of my membership.

Membership Terms

Month-to-Month Membership

Automatically renews each month.

May be canceled at any time with 30 days written notice.

Six-Month Membership

Minimum commitment of six (6) months.

May be canceled after completing the initial three (3) months with 30 days written notice.

Twelve-Month Membership

Minimum commitment of twelve (12) months.

May be canceled after completing the initial eight (8) months with 30 days written notice.

Membership Pause

We understand that life circumstances change.

Members may pause their membership for up to three (3) consecutive months due to circumstances such as:

Pregnancy or childbirth

Extended travel

Medical recovery

Other significant life events approved by Misama

Pause requests must be submitted in writing before the requested pause date whenever reasonably possible.

Memberships will automatically resume at the conclusion of the approved pause period.

Membership Changes

Misama reserves the right to modify membership pricing, benefits, or policies for future memberships.

Any changes will not affect prepaid Founding Member memberships during their initial twelve-month membership term.

Member Acknowledgement

By signing below, I acknowledge that:

I have read and understand the Misama Wellness Membership Agreement.

I understand that this membership is designed to support long-term wellness and is not intended to

replace a recommended physical therapy plan of care when higher treatment frequency is clinically appropriate.

I authorize Misama LLC to automatically charge my payment method according to the membership selected.

I understand that I am responsible for maintaining a valid payment method on file and will provide updated payment information within five (5) business days should my payment method expire or become

invalid.

I understand and agree to the cancellation, pause, and billing policies outlined in this agreement.
