Blood Sugar, Rapid Weightloss Medications & Fainting Risk

I acknowledge that tattooing may cause physical stress, discomfort, or drops in blood sugar, which may result in dizziness or fainting. (If diabetic, I am aware that my A1C levels are with in safe range for my appointment today. I understand that GLP-1 medications, rapid weight-loss medications, or supplements , as well as inadequate food intake, may increase this risk. I confirm that I have eaten prior to my appointment and will notify my artist immediately if I feel unwell. I agree that Purple Ink Studio is not responsible for fainting or related incidents including any additional fees accrued due to fainting. *

Blood Thinners & Supplement Disclosure

I understand that certain medications and supplements may affect bleeding and healing, including but not limited to: • blood thinners• aspirin / NSAIDs • fish oil • vitamin E • herbal supplements • alcohol I confirm I have disclosed relevant use and understand this may impact my tattoo results. I agree that Purple Ink Studio is not responsible for outcomes affected by these factors. *

Tattoo Longevity & Appearance

I am prepared for my tattoo to change in appearance as time goes on due to my body and external means, but not limited to, naturally pulling apart the ink, sun exposure damage, normal wear of the skin in said area, etc. My artist has informed me that she cannot guarantee the longevity of my tattoo with my choice of placement due to being a high use area, delicate type of skin or additional reasons. Examples of many “no guarantee” tattoo placements are fingers, palms, between fingers/toes, inner lips, feet, etc. I am aware that their are NO free touch ups with a “no guarantee” tattoo placement. *

Artistic Variations

I understand that tattooing is an art form and that each tattoo is custom and applied by hand. I acknowledge that the final result may vary slightly from reference images, drawings, or stencils due to factors including but not limited to: • skin texture and tone • body contours and movement • placement on the body • the tattooing and healing process I trust the artist’s professional judgment and understand that these variations are a natural part of the tattoo process. *

From Idea to Skin

I am aware that tattoos may vary from the art itself to the tattoo on skin. I understand that the tattoo is layers beneath my skin which may alter the colors of the end product due to my skin tone being a filter. *

Purple Ink Studio is not responsible for the meaning of symbols or spelling of text that I have given them. *

Aftercare

I have been given the care instructions and all questions have been answered to my total satisfaction in regards to the process of my tattoo and healing after. I acknowledge that the risks (known and unknown) may include, but are not limited to, scarring, infection, difficulties detecting skin diseases, allergic reactions to the tattoo pigment or bandaging, soap, latex gloves, etc. *

Touch- Up Policy

I understand that a touch-up may be needed after my tattoo has fully healed. (Touch-ups are limited to minor refinements and do not include redesigns or significant changes.) One complimentary touch-up may be offered at the artist’s discretion within 6–8 weeks of the original appointment, provided that: • the tattoo has been properly cared for according to aftercare instructions • the skin has fully healed • the touch-up is scheduled within the required timeframe I understand that touch-ups requested after this timeframe, or due to factors such as improper aftercare, skin type, lifestyle, sun exposure, or placement (including “no guarantee” areas) may be subject to additional fees. *

Sterilization & Safety

I understand that Rachel Purple Tattoos, LLC. follows proper sanitation and safety procedures in accordance with state and local health regulations. I acknowledge that all equipment used during my procedure is either single-use and disposable or properly cleaned and sterilized, and that appropriate barrier protection and hygiene practices are used throughout the tattoo process. *
