Breathwork Liability & Release Waiver

We prioritize the safety and well-being of all our participants, and as part of our commitment to ensuring a secure environment, we require the completion of this Liability Waiver Form.

Health Conditions

A breathing session may not be suitable for you if you have the following conditions:

Cardiovascular problems, abnormally high blood pressure, aneurysms, epilepsy, or a history of seizures
Use of heavy medication
Severe psychiatric symptoms, especially psychosis, paranoia, or bipolar disorder
Osteoporosis, recent surgery, glaucoma, or pregnancy
Asthma (participants should bring their own inhaler and consult with their physician and breathing session instructor before participating)
Emotional or spiritual crisis, or untreated mental illness without adequate support

This list is not exhaustive. If you have a condition not listed here or any medical concern, you are strongly advised to consult with a physician before participating in breathing sessions.

Certification of Good Health

By signing this agreement, I warrant and represent that:

I am in good physical, mental, psychological, and emotional health.
If I am not in good health, I understand and agree that I will not be allowed to perform the activities and sessions.
My declaration of good health is a material agreement permitting me to participate in the breathing sessions.

Acknowledgement of Risks

I acknowledge and understand that:

The facilitator(s) are not doctors, psychiatrists, or licensed health care specialists.
The activities offered are not intended to treat, diagnose, or cure any medical condition, whether physical, psychological, or emotional.
Participating in breathing sessions involves inherent risks, which may include physical injury, emotional distress, or other unforeseen consequences.
I voluntarily assume all risks associated with my participation, including but not limited to those caused by negligence.

Release of Liability & Indemnification

In consideration of my voluntary participation, I agree to the following:

I release and discharge 5D Sound Space and Raven Feurer, their agents, employees, or representatives, from any and all liability, claims, demands, or causes of action arising out of or related to my participation in these activities.
I agree to indemnify, defend, and hold harmless 5D Sound Space and Raven Feurer, their agents, employees, or representatives, from any claims, damages, liabilities, and expenses arising out of or related to my participation in these activities.

By signing below, I acknowledge that I have carefully read this agreement, fully understand its terms, and agree to be bound by them. I confirm that I am signing this agreement freely and voluntarily, with full knowledge of its significance.
