Agreement
By submitting this enrolment form, you acknowledge and agree to the terms outlined herein, effective immediately upon submission.
Fees
All fees must be paid prior to or on the day of the first lesson via cash, EFTPOS, or fortnightly direct debit. In the case of unpaid or overdue fees, EBA reserves the right to engage a debt collection agency. You will be responsible for any associated collection agency or administrative fees.
Refund Policy
EBA does not provide refunds if a student withdraws from a course after the term has commenced. In cases of absenteeism, you are welcome to attend a make-up session within the same term, subject to availability. Refunds will not be issued for missed lessons or changes of mind.
Uniform and Grooming
Students are required to wear the designated EBA uniform each week. Loose jewelry is not permitted, and hair must be styled appropriately: a bun for ballet classes and a sleek ponytail for other styles. Students not in uniform will not be permitted to participate in class.
Child Collection
Parents or guardians must collect students promptly at the conclusion of their class. EBA does not provide supervision in the waiting room.
Physical Contact
Parents acknowledge that some physical contact may be necessary during classes to demonstrate exercises, correct techniques, or administer first aid when required.
Working with Children
All EBA staff hold valid Working With Children Checks to ensure the safety and wellbeing of all students.
Communication
All communication with EBA should be directed via phone (0410 500 211), email ([email protected]), or in person at the reception desk. Teachers should not be contacted directly or personally.
First Aid
In the event of an emergency where parents or guardians cannot be reached, EBA staff may call an ambulance. By enrolling, you consent to this action if deemed necessary.
Cancellation of Enrolment
EBA reserves the right to cancel enrolment if any of the following occur:
a) Outstanding invoices remain unpaid.
b) Non-compliance with the Terms & Conditions.
c) Failure to follow reasonable directions from staff, compromising the safety or wellbeing of staff or students.
d) At EBA’s sole discretion, if it is deemed inappropriate to continue providing services to the enrollee.
Suspension of In-Person Classes
Should government-imposed closures require suspension of in-person classes, lessons will transition online via Zoom. Fees will remain unchanged, and no refunds or credits will be issued. By enrolling, you accept the potential for such a transition. For those unable or uncomfortable attending live Zoom sessions, pre-recorded classes will be provided as an alternative.
Applicable Law
This agreement is governed by the laws of Western Australia and any applicable Commonwealth legislation.
