Thank you for your interest in a Jolis Collie. This application is the first step in what we hope will be a long and wonderful relationship — not just between you and your dog, but between you and us.

We take placement seriously. Every question on this form helps us understand your home, your lifestyle, and what you are truly looking for — so we can match you with the right dog at the right time. There are no wrong answers. Honesty is always the right answer.

Once we receive your application we will review it carefully and be in touch within five to seven days. We may follow up with additional questions or schedule a phone call before making any decisions. We also ask that you submit photos of your home and yard where the dog will live — you can email these directly to [email protected].

GROOMING

The Collie coat requires regular maintenance and we want to make sure every family feels prepared and confident. There are no wrong answers here — just honest ones, and we will make sure you have everything you need.

Do you plan to use a professional groomer, maintain the coat yourself, or a combination of both? *

Are you comfortable with regular at-home brushing between grooming appointments? (We recommend a conditioning spray and brush-through once or twice a week, plus a professional bath and blow dry every six weeks.) *

Are you prepared to invest in basic grooming tools — a pin brush, slicker brush, wide tooth comb, and conditioning spray? *
