My Beauty Truck Waiver

All girls MUST have a waiver to participate:

Each individual participant (or authorized guardian, if minor) must read and agree to this release of liability prior to participating in any My Beauty Truck (MBT). You will give your daughter permission to participate in My Beauty Truck activities and agree to these terms by checking the box that says "I agree to the terms & conditions"

Product Usage:

By signing this permission slip, you acknowledge your understanding of MBT activities which may include the use of beauty and skin care products. MBT is not responsible for any injuries or allergic reactions that happen as a result of using products during activities.

Photos:

Participants may have their photos taken during MBT events. Photos may be uploaded to a private online gallery and/or social media sites and shared amongst guests and their friends. MBT is not responsible for how photos are shared. By signing this form you are acknowledging that photos and videos may be taken of your child. You acknowledge that MBT is not responsible for how photos are shared.

Accidents:

In order to board the My Beauty truck, it is necessary to use stairs. MBT is not responsible for any damages, accidents or injuries that may incur while under our supervision onboard or property surrounding the My Beauty truck.

Age Allowance:

The youngest age we serve is 7. Girls younger than 7 will not be able to participate in MBT activities.

Release of Liability:

In order to participate in MBT events or activities, I, the undersigned, freely assume all risks, both known and unknown, and assume full responsibility for my and my child's participation and safety. I, for myself, and on behalf of my heirs, assigns, personal representatives and next of kin hereby release and hold harmless MBT their officers, officials, agents or employees, from any and all liability for injury, loss or damage to personal property. I acknowledge, understand and agree that I have read this release of liability and assume all risk associated with participating and that I sign this release of liability voluntarily and without inducement.
