Release of liability Form

It is important to note that your participation in the Aikido Seminar offered by the Aikido Training Academy is entirely voluntary. This seminar involves potentially hazardous physical activity and contact. Therefore, you confirm that you are in good physical condition and have no medical or physical conditions that would restrict your participation in the Aikido Seminar.

You hereby assume all risks of injury that may result from your participation in such activities and release and discharge Aikido Training Academy, its owners, instructors, employees, and agents from any and all liability, claims, demands, and causes of action that may have arising out of or related to any injury, disability, death, or loss that you may sustain as a result of your participation in the Aikido Seminar.

It is crucial to understand that the practice at the Aikido Seminar involves physical contact and, despite the best efforts of Aikido Training Academy to prevent injuries, there is a risk of injury or harm that cannot be eliminated. Therefore, you are committed to complying with all rules and regulations of Aikido Training Academy and to following the instructions of the instructors.

You understand that this release of liability form is a legally binding agreement and that you are waiving your right to bring any legal action or claim against Aikido Training Academy, its owners, instructors, employees, and agents as a result of your participation in the Aikido Seminar.

You agree to allow the following at the Aikido Seminar: video, images, and audio of yourself. You understand and agree that videos, images, or audio of yourself may be used to promote future Aikido seminars, events, workshops, and teaching material to help in the education of Aikido. You agree to be notified of upcoming Aikido events, seminars, and workshops.

You agree that you have carefully read and fully understand the contents of this release of liability form. Your signature below confirms you agree with all terms and conditions within the release of liability.
