MEDICAL AUTHORIZATION AND LIABILITY RELEASE

I hereby acknowledge that I am participating in activities provided by PYRO Cheer & Dance. In consideration of being permitted to participate in these activities, I agree to release and discharge PYRO Cheer & Dance, its owners, employees, instructors, agents, and volunteers from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my participation in the activities provided by PYRO Cheer & Dance.

I understand and acknowledge that participating in cheer and dance activities involves certain risks, including but not limited to bodily injury, property damage, or even death. I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury that may be sustained by me or my minor, or any loss or damage to property owned by me or my minor, as a result of participating in these activities.

I hereby agree to indemnify and hold harmless PYRO Cheer & Dance, its owners, employees, instructors, agents, and volunteers from any and all claims, demands, losses, causes of action, liability, damages, or costs, including but not limited to attorney fees and other legal expenses, which may arise out of or relate to my participation in the activities provided by PYRO Cheer & Dance.

I have read this release of liability and fully understand its terms. I understand that I am giving up substantial rights, including my right to sue, and I acknowledge that I am signing this document freely and voluntarily, without any inducement or assurance of any nature.
