PARENTAL PERMISSION FORM

Messiah invites your youth to have a wonderful experience at Camp Yeshua this summer. To make our camp run as smoothly as possible and to avoid confusion, we would like to clarify some points with you.

• Make sure your youth WANTS to be at Camp Yeshua. Please do NOT send us an unwilling camper. Please inform us of any situation/difficulty/conflict/etc. about your youth so we can provide the best experience possible for them and others.

• All campers must be pre-registered. Please make sure the registration form is filled out completely. After the completed form and deposit are received, a confirmation email will be sent. A full confirmation packet will be sent before the event with maps and a list of what to bring to camp.

• Your youth must be physically able to participate in summer camp activities. Some activities, by their nature, are physically and mentally demanding and subject to hazards that cannot be foreseen or prevented. Your youth is responsible for their participation in such activities and assumes all ordinary risks associated with these activities which may include injury. Please disclose ALL health-related issues (including special dietary requirements) so our medical staff can best assist your youth's participation in Camp Yeshua.

• Cell phone usage by youth campers is prohibited during Camp Yeshua. Should you need to contact your youth during that time, contact information to our staff will be provided for you in your confirmation packet. If you have an issue with no contact for five days, please let us know.

• Your youth's likeness may be used in the promotion of Camp Yeshua in the form of photos, video, and/or written copy regarding their participation in Camp Yeshua.

• Your youth needs to read, understand, and sign the Youth Contract that follows. Please be aware of its contents as well.

• As a parent, you will not show up at the campground during the event.

Parent / Guardian Agreement

I have read and understood the above information.

I hereby authorize treatment for my youth {name} while at camp by any doctor, nurse, or hospital as deemed necessary by camp authorities in the event of an accident, injury, or illness, and I, the legal guardian of said youth, assume the responsibility for all medical bills.

I give permission for my youth to ride in any vehicle designated by the adult in whose care he/she has been entrusted while attending and participating at Camp Yeshua.

I certify that my youth is in good physical condition and is able to participate in all camp activities, unless specifically noted on the registration form.

Code of Conduct

YOUTH CONTRACT

One of the goals of Camp Yeshua is to provide a fun and safe environment for Messianic youth to congregate and leave the secular world behind. It is for this reason that we have some rules that must be followed to ensure that everyone at Camp Yeshua will have a fun and blessed time. The following rules will be discussed in detail on the first day of camp.

Prohibited Possessions

  Tobacco in any form
  Alcohol
  Drugs
  Weapons of any kind
  Explosives of any kind
  Any items with direct ties to the occult (Magic: the Gathering, jewelry, etc.)

Prohibited Actions
  Blatant disregard for rules and authority
  Physical altercations (fighting)
  Stealing
  Trespassing another's property
  Sexual contact with anyone: camper, including public display of affection
  Bullying of any kind
The possession or action of any of the above items
Items not to be Used

 Cell phones*
 Pocket knives / multi-tools  MP3 players**
 Video game systems
 Tablets / E-readers
 Laptop computers
 Skateboards, Skates, etc.

If you come with any of these items, we will hold them for the duration of camp and return them at the end of camp. (We are not responsible for lost or stolen items.)

* Use of cell phones by campers during Camp Yeshua is prohibited. For this reason, cell phones will be turned in to the staff at check-in. They will be kept safe and returned at the end of Camp Yeshua. If you need to contact your parents or there is some form of emergency, talk to a member of the adult staff. By signing below, you agree to this policy. If you are found to have such a device after check-in it will be confiscated. 

The possession or actionof any of the above will result in disciplinary action, up to and including immediate dismissal from Camp Yeshua without refund, removal from the premises of Camp WoW, and/or a ban from subsequent Camp Yeshua events at the discretion of the Camp Director.

The use of foul language will not be tolerated in any form. It will be the judgment of designated adult staff member to determine what is inappropriate. Fighting or activity that has the intention of causing harm to another person will not be tolerated.

Modesty will prevail in all clothing and behavior matters. Likewise, any questionable slogans or graphics on clothing will not be allowed. It will be the judgment of designated adult staff member to determine what is inappropriate. For your safety, closed-toe shoes are to be worn at all times. During swim activities, girls are to wear a dark t-shirt over any two-piece bathing suit and boys are to wear a t-shirt or tank top with swimsuits.

You will not be allowed to leave the camp facilities for any reason without the approval of the operations manager. You must also comply with further guidelines that will be announced on the first day of camp. You will not be able to get into your car and will have to turn in your car keys at registration.

MP3 players may be used if you are participating in Davidic dance.

Youth Agreement 

I understand and agree to obey the rules of camp and conduct myself in a manner that is moral and upright before the Lord. I will follow the camp schedule and participate in all camp activities to the best of my ability. I will respect all “off-limit” areas and each nightly curfew. I will not be alone with any member of the opposite sex at any time during Camp Yeshua. I understand that the six days of Camp Yeshua are to be a departure from the secular world where I live. I will not say, wear, nor do anything that may offend any other person at Camp Yeshua. I will do my best to meet new friends and be a blessing to everyone at camp. Most of all, I will have fun and receive a blessing from Camp Yeshua!

Assumption of Risk and Release of Liability

The undersigned acknowledges that during the session that the applicant has requested to participate in, Certain risks and danger may occur. The undersigned recognizes that such risks and danger may include Loss or damage to personal property, physical or psychological damage and/or injury, not excluding fatality due to accident. I certify that I am completely healthy (both physically and emotionally) and capable of participating in this session. I have listed on the medical information form medical conditions That WALK ON WATER Inc. should be aware of which may hinder my participation in the session. However, I understand that it is solely my responsibility to determine whether there is any medical reason That I should not participate in the session and to obtain approval for any and all activities from the appropriate Health-care providers. The health history is correct as far as I know, and the person herein described has permission To engage in all prescribed camp activities except as noted. I hereby authorize the medical personnel selected by The camp director and/or church leader to order x-rays, routine tests, treatment, and necessary transportation for Me/my child as deemed necessary. I, individually and on behalf of the minor and all other family members, Executors or administrators, do hereby release, forever discharge, and agree to hold blameless WALK ON WATER Inc. and its counselors, staff, employees, agents, and lessors from any and all liability, claims, INCLUDING, BUT NOT LIMITED TO THE NEGLIGENCE OF WALK ON WATER Inc. STAFF, DIRECTORS, COUNSELORS, EMPLOYEES, AGENTS and LESSORS, or demands for personal injury, sickness, or death, as well as property Damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the participant While said person is participating at WALK ON WATER. In consideration of, and as part payment for, the Right to participate in such a program and the services arranged for me by WALK ON WATER Inc. its staff, Directors, counselors, employees, agents and lessors, from any and all liability, actions, causes of action, INCLUDING BUT NOT LIMITED TO THE NEGLIGENCE OF WALK ON WATER INC DIRECTORS, COUNSELORS, EMPLOYEES, AGENTS and LESSORS, debts, claims, and demands of every kind and nature Whatsoever, whether for bodily injury, property damage or loss otherwise, which I now have or which may arise From or in connection with my program or participation in any other activities arranged for me by WALK ON WATER Inc. its staff, directors, counselors, employees, agents, and lessors, for all members of my family, Including any minors accompanying me. I SPECIFICALLY AGREE THAT MY AGREEMENT TO INDEMNIFY AND HOLD HARMLESS WALK ON WATER INC. ITS STAFF, DIRECTORS, COUNSELORS, EMPLOYEES, AGENTS and LESSORS, INCLUDES ALL LITIGATION COSTS AND ATTORNEY FEES FOR ANY LITIGATION BROUGHT ON BY MYSELF, ON BEHALF OF THE MINOR, IF APPLICABLE, OR ANY OTHER FAMILY MEMBER. I grant permission to WALK ON WATER to use photographs and any video taken by WALK ON WATER for use on web sites or other electronic form or media, without notifying me. I hereby waive any right to inspect or approve the photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photographs. I hereby agree to release and hold harmless WALK ON WATER, via electronic or media, from and against any claims, damages or liability arising from or related to the use of the photographs, including but not limited to any re-use, distortion, blurring, alteration, optical illusion or use in composite form, either intentionally or otherwise, that may occur or be produced in production of the finished product. I also state that I am not under, and will not be under the influence of any Chemical
substance including alcohol. I fully understand that my physical activity involves risks of injury. I also understand that my participation in this WALK ON WATER Inc. program is entirely VOLUNTARY. I enter into this session and take full responsibility for my decision to participate or not to participate and agree To follow all safety instructions.
