I understand that by submitting this form, my student will be unenrolled from the above-listed Ovation Dance Class.
I understand that upon submission of this form, my 30-day notice period will begin.
I understand that automatic payments will stop after the one-month (30-day) notice period (unless an arrangement has been made with the director) and any remaining tuition, registration, costume fees, and/or balances have been received.
I understand that there are no refunds on any tuition, registration, competition, recital/performance, or costume fees already paid. The exception to this is for tuition, if this form is submitted during the 1st 2 weeks of the Fall-Spring season.
I understand that Costume/shoe Fees are nonrefundable. Class changes before October 1 will result in studio account credit for these payments made. Changes after October 1 will require the full costume to be paid for before class changes or withdrawals can be finalized.
I understand that once a costume has been ordered, it belongs to my family upon payment of all outstanding balances, including any applicable withdrawal fee. If these payments are not received, Ovation Dance will retain the costume.
I understand that by submitting this form, my student will be ineligible to return to this class unless enrollment for the class is open.
I understand that upon notification of withdrawal, a $30 withdrawal fee will be charged to process my withdrawal request if this is after the two-week period from the start of the Fall-Spring season.
Important: Failure to submit this form or to make the necessary payments will result in the withdrawal request not being processed. As a result, the student will remain enrolled in the class and continue to be responsible for all applicable tuition and fees until a proper withdrawal is completed.
