I ______________, hereby give my consent to participate in the Rise and Reach Project (the "Organization"). I understand that I will engage in various activities, programs, and events organized by the Organization. I acknowledge and understand the nature of the activities that I will participate in, including but not limited to mentorship, recreational events, fundraising initiatives, and other activities determined by the Organization. I am aware that I may be interacting with mentors, volunteers, and other participants during these activities. I certify that I am physically, mentally, and emotionally capable of participating in the Organization's activities and that I have no medical or other conditions that would prevent or limit my participation. I understand that it is my responsibility to inform the Organization of any changes in my health or medical condition that may affect my participation in the activities. I acknowledge that participation in the Organization's activities involves inherent risks, including but not limited to the risk of injury, accidents, or illness. I understand that the Organization takes reasonable measures to ensure the safety and well-being of its participants, but I recognize that accidents or incidents may still occur despite these efforts. I authorize the Organization and its officers, directors, employees, and agents to take any reasonable measures necessary to protect my health, safety, and welfare during my participation in the Organization's activities. This may include obtaining emergency medical treatment for me, as deemed necessary by the Organization, and I agree to be responsible for any medical expenses incurred on my behalf. I release and hold harmless the Organization, its officers, directors, employees, and agents from any and all liability arising from or related to my participation in the Organization's activities, including but not limited to any injury, loss, or damage incurred by me during my participation. I have read and understand the nature of the Organization's activities and hereby give my consent to participate in the Rise and Reach Project.
