Refund Policy
I understand and accept that the registration fee is non-refundable. This fee secures tournament entries, professional instruction, facility access, uniforms, insurance, and administrative costs. I acknowledge that the Glaciers may review refund requests in extreme hardship situations at the discretion of the Board of Directors.
Affidavit of Insurance, Hold Harmless, Parental Conduct Agreement & Itinerary Change-2026
I, the parent or legal guardian of the aforementioned player above ("Player"), a minor participating as a "participant/player" on an Ohio Glaciers/Glacier Sports, Inc. baseball team (“Glaciers”) located in Mahoning County, Ohio, acknowledge that, in consideration of the Glaciers permitting Player to participate in the Glaciers travel baseball schedule for the 2026 Season, there is accident and health insurance coverage in place for Player that will remain effective throughout the 2026 season, and agree to maintain this coverage for the duration of the season.
I agree to indemnify, protect, and hold harmless Glaciers, along with its officers, board members, supervisors, agents, assigns, independent contractors, servants, employees, and all service providers—including those instructing, supervising, or chaperoning players/participants before and during the season—from any claims or liabilities, including but not limited to personal injury, property damage, loss, court costs, attorneys’ fees, and interest, arising from participation in scheduled practices or games.
As a member of the Glaciers Team, I acknowledge that participation includes travel to multiple tournaments, which may require overnight stays at accommodations selected by the team manager. I also understand that the Glaciers organization, its officers, agents, and employees reserve the right to end a minor’s participation if there is non-compliance with organizational conduct regulations, failure to follow the directions of team staff or chaperones, or actions judged by the board or its representatives to be inconsistent with the interests or well-being of the team. In such cases, the minor will be sent home at their own expense.
I acknowledge that the Glaciers organization, including its board, officers, agents, and employees, reserves the right to make cancellations, modifications, or substitutions at any time prior to or during the season in response to emergencies, changing circumstances, or in the best interest of the team.
In exchange for my child being allowed to join and play for the Ohio Glaciers Baseball Team, and in recognition of the efforts and authorization provided by management, coaches, agents, and other participants in starting and running the team, I agree to the following terms:
I hereby acknowledge and fully understand that both my child, as the participant, and myself, whether as an assistant or spectator, will be participating in activities that involve inherent risks of injury. Such risks may arise not only from our own actions, inactions, or negligence, but also from the actions, inactions, or negligence of others, as well as from the condition of the premises or land where games and practices are conducted, or from any equipment utilized. Additionally, I recognize that there may exist other risks currently unknown or not reasonably foreseeable. These risks may include, but are not limited to: general risks associated with the activity; latent or apparent defects or conditions in equipment or property provided by GLACIERS or third parties; actions or omissions of other participants; my own physical condition; the state of the facilities used by GLACIERS and their surrounding grounds or terrain; and first aid, emergency treatment, or other services rendered during or following any practice, game, or team event.
I acknowledge and accept all the risks associated with this activity. My participation is voluntary, and I recognize responsibility for any injury or damage that may occur to myself or my child as a result of participating.
On behalf of myself, my child, my heirs, assigns, and personal representatives, I hereby voluntarily release, waive, discharge, and agree to indemnify and hold harmless GLACIERS, as well as its officers, board members, supervisors, agents, assigns, independent contractors, servants, employees, managers, coaches, and all individuals or organizations providing services to Glaciers (“Released Parties”). This also includes, where applicable, operators or lessors of premises used for Team events or activities. This release applies to any and all liability, claims, demands, or causes of action arising from, or in any way related to, my participation in a Glacier’s Baseball Team or my use of GLACIERS’ equipment or facilities.
I certify that I possess adequate insurance coverage, including both medical and liability insurance, through health insurance and homeowner’s liability policies, to cover any potential injury or damage resulting from my participation in these activities. Alternatively, I agree to personally bear any associated costs. Additionally, I confirm that my child does not have any medical or physical condition that would impede their safe participation in this activity. Should any such condition exist, I accept full responsibility for the potential risks and costs involved.
I hereby provide GLACIERS permission to administer basic First Aid to myself and/or my child, and I further authorize GLACIERS, and its agents or employees, to summon 911 or other emergency personnel as GLACIERS, in its judgement, deemed necessary or appropriate. By signing this document, I acknowledge that if I am injured or damaged, or my child is injured or damaged during participation in this activity, a court of law may find me to have waived my right to maintain a lawsuit against Ohio Glaciers Baseball Team, Glaciers Sports, Inc., and each of the other parties listed in the Agreement.
I hereby grant GLACIERS permission to use my name, image or likeness in any photographs and/or video of myself or my child in publications, brochures, new releases, online, and in other communications or written material related to and/or to promote/market Glaciers Sports, Inc.
Child Abuse Prevention Policy & Procedures:
Statement on Child Abuse Prevention: Glaciers Sports, Inc., as an organization working with youth, prioritizes the safety and wellbeing of participants in its programs. The organization prohibits bullying, abuse, and other forms of violence, and addresses reports of such conduct in accordance with established policies. Concerns or issues may be reported at any time, and the organization will respond to them in a fair and balanced manner.
Policies & Procedures:
Glacier Sports, Inc. has established the following Policies and Procedures to ensure a safe environment for all members, athletes, and coaches. The Board of Directors will conduct an annual review of these policies and update them as necessary, including revising definitions of prohibited and discouraged behaviors.
The Coach or Trainer recruitment process will consist of the following steps:
Background Check/Screening – All prospective coaches and trainers will undergo a background check conducted by an authorized third-party vendor. This screening will include thorough inquiries into any prior incidents involving sexual abuse or other unlawful activities.
An annual Abuse Prevention Orientation will be conducted.
A designated member of the Board of Directors shall review this policy with all coaches and trainers and each coach and trainer will certify their acknowledgment and acceptance of our abuse prevention policies.
At the beginning of the season, the policy will be provided or communicated to parents.
Establishing Criteria for Prohibited Conduct
Prohibited Behavior
Use of inappropriate or disrespectful language. Coaches and trainers are expected to intervene and address any instances of unprofessional language or conduct among team members, including remarks of a sexual nature or harassment.
Threats to inflict physical injury upon a minor or intentionally causing physical harm to a minor.
Committing any sexual offense against a minor or engaging in any sexual contact with a minor.
Making any kind of sexual advance, or making a request for sexual favors, or engaging in other verbal, visual or physical conduct of a sexual nature.
Reporting of Suspected Child Sexual Abuse
The Board of Directors will designate at least one member of the organization to receive reports of sexual abuse or other inappropriate conduct and to take appropriate action upon receiving such reports.
Volunteers, parents, and program participants are instructed to report any observed or reported incidents of abuse or suspected abuse to the designated representative or any member of the Board of Directors. They may also choose to notify the relevant law enforcement agencies directly.
The designated member of the Board of Directors shall promptly inform the appropriate law enforcement authorities upon receipt of a report concerning a potential incident of abuse.
Should a suspected case of abuse be reported, the coach involved may be suspended from their duties pending the outcome of an investigation.
When deemed necessary, the Board of Directors shall communicate reports of child sexual abuse to league members. The confidentiality of individuals who submit such reports will be strictly maintained.
Rules and Policies Applicable to All Glaciers Teams
All players and teams must adhere to established rules and policies set forth below to qualify for participation in Glacier Baseball competition: Participants are required to maintain appropriate conduct throughout the event.
Players are strictly prohibited from purchasing, distributing, or using alcohol, tobacco, or any other drugs without the written consent of a licensed, practicing physician.
The use of inappropriate language, whether on or off the field, is strictly prohibited. Any violation will result in a suspension from one game.
The throwing or misuse of equipment is strictly prohibited. Should any equipment be damaged because of such actions, the player will be responsible for reimbursing Glacier Sports, Inc. for the full replacement cost.
Uniforms must be clean, properly maintained, and tucked in when required; from calisthenics through the end of the game, uniforms are to be worn as specified. Only Glacier Board-approved uniforms may be used during games.
Hats are to be worn in a straightforward manner when required.
Players are expected to move quickly to and from their positions. Players should run at full speed on all ground balls and fly balls, regardless of the likelihood of a putout.
Only ice water or sports drinks are permitted in the dugout during games; outside food and other beverages are not allowed.
Spectators are not permitted in the dugout during a game. This applies to parents, family
members, friends, girlfriends, scouts, and any other non-players. Additionally, non-players are not allowed to visit with players during a game.
Players may leave the dugout during a game for purposes such as retrieving a foul ball, using the restroom, warming up, stretching, throwing, or in cases of medical or weather-related emergencies. Players should complete the required activity and return to the dugout promptly. Any other requests to leave the dugout should be discussed with and approved by a member of the coaching staff.
All players are required to help carry equipment for practices and at games.
Altercations between team players or opposing players are discouraged and prohibited. While tempers can flare unexpectedly, any fighting will be disciplined by immediate suspension and probable dismissal from the team.
All players/guardians are expected to call a member of the coaching staff in the event of a practice or game is to be missed. If a player is absent without permission, the minimum penalty is an immediate suspension with a maximum penalty of dismissal from the team.
For those athletes who compete in more than one sport which requires additional training/games during the baseball game season, the following applies: Training/game activities should understand that for each game/practice that is missed because of the other sport, the player will be benched the same number of games. During the baseball season, the program assumes Glacier Baseball is your priority. If you know there is an unavoidable conflict during the baseball season between another sport commitment and the Glacier Baseball season, (April 1st through July 31st ) please do not tryout. You will not be rostered.
Players are expected to always uphold Good Sportsmanship. In short: players will keep their temper regardless of victory or defeat, be good sportsmen and never heckle opposing players, umpires, or spectators. Unless otherwise indicated, the penalties for any of the rule violations will result (at a minimum) in an immediate suspension of practice/playing privileges, and a maximum penalty of dismissal from the team. The coaching staff will decide the severity of the penalty(ies) to be imposed for any rule(s) that is violated and/or for inappropriate behavior. All coaching staff disciplinary decisions are final.
Parents, family members, or friends are not permitted to heckle, harass, or use derogatory, profane, or vulgar remarks or gestures toward the opposing team, coaches, or umpires. Individuals who violate this rule will be asked to leave the ballpark immediately. A second violation will result in removal from the park and exclusion from other team activities. A third violation may lead to the player's suspension or dismissal from the team. If a player is ejected from a baseball event for "unsportsmanlike conduct," a two-game suspension will be imposed. Before returning to competition, the player, head coach, board member, and the player’s parent or legal guardian will meet to discuss the incident and determine any further action. A second suspension for "unsportsmanlike conduct" will result in loss of participation privileges in that sport for the remainder of the season and will require another meeting between the parties involved to address the appropriate steps.
Because we are concerned with the safety & progress of our teams and players, no Glacier player is allowed to play for another baseball team during the Glacier baseball season, unless approved by their team manager. Violation of this rule will result in the player’s suspension and/or dismissal from the team.
Glacier organization fundraising is important to keep the cost lower for our players by helping us pay expenses such as insurance, website, equipment, field and indoor maintenance, field and facility improvements, baseball supplies, administration costs, legal costs, training and coach expenses. Each team is responsible for actively participating in the fundraising either monetarily or volunteering their time or both.
The Board Members and/or coaches retain the right to use discretion in the enforcement of the rules, policies, and consequences due to a broad range of behaviors, mitigating circumstances and a sincere desire to modify behavior.
Any Glacier player who voluntarily decides not to participate on the team during the season (November 1st thru July 31st) after making the team will be ineligible to participate on any other Glacier team during that season and 1 season thereafter, including practices and any other activity sponsored by the Glaciers.
If your child is selected to participate as a member of any Glacier team, you agree to pay a Glacier registration downpayment fee. The balance of the Glacier Registration Fee must be paid by the first day of November preceding the season of participation. Because we utilize the Glacier registration fee to secure the services of professional instructors; operation, maintenance and use of the Glacier indoor athletic facility & outdoor fields, tournaments, insurance, uniforms, capital improvements, and administrative costs, the Glacier registration fee is non-refundable. By registering and paying the Glacier registration fee or any portion thereof, you hereby acknowledge and agree that said payment is non-refundable. There are limited circumstances in which the Glaciers may consider a refund of all or a portion of the Glacier Registration fee due to a hardship suffered by the participant. A review of a request for a refund will be conducted by the Glaciers Board of Directors.
All Glacier players are expected to abide by their coaches’ rules concerning conduct traveling to, at and returning from a baseball event. These include practices, games and any other activity sponsored by the Glaciers. A coach is entitled to require rules that go above and beyond the above set of guidelines as well to impose any reasonable consequences for inappropriate behavior.
The Glacier athletic participant and parents acknowledge that participation on the Glacier baseball team is a privilege and not a right. We further agree and acknowledge that playing time, position played, and level played are determined by the coaching staff and not to challenge the coaches or board members as to these determinations. All participants are subject to the rules & policy listed above and by our signature we hereby acknowledge having read and agree to them.
Glaciers Indoor Facility Rules
Food & beverages are only allowed in the vending area. Water is allowed in the dome.
All players in the batting nets must wear a baseball helmet.
All coaches/volunteers/trainers pitching batting practice must wear a baseball helmet.
No players/and or minors allowed to pitch batting practice.
Only players, coaches, and trainers are allowed in the dome during practice times. There are 24-hour security cameras in both the dome and the batting facility. Non-Players, non-coaches, and non-trainers can sit in the vending area of the batting facility and view the dome practice on the wall mounted television screen.
No chairs allowed in dome or batting nets.
Baseballs cannot be removed from the batting nets and/or dome.
No hitting into the fences of the batting tunnels.
Teams are responsible for cleaning up baseballs, picking up trash, and put back nets in their designated areas after their practice times.
Lockers are to be used by the head coach and trainers. One locker per team.
Any holes in net and/or L screens must be reported to Glacier management.
All non-players must be seated in the vending area in the batting barn.
Bat bags and team equipment must be hung on the batting barn fence or behind nets in the dome.
No carrying or swinging bats outside the batting cages.
No live hitting inside the dome. The dome can be used for defense, base running, and bunting.
No scrimmages or games inside the dome.
Only Glacier registered players and players getting training from Glacier Authorized trainers are allowed to practice or train within the glacier facility.
Any injuries in the Glacier facility must be reported immediately to Glacier management.
Teams must have at least one background check approved coach with their batting, team, and camp practice times.
No Horseplay in the Glacier facility.
No tobacco, guns, drugs, or alcohol allowed within Glacier facility or on the premises of any Glacier fields or SR Products facility.
Please keep restrooms clean
Help to shovel the walks during snow accumulation. Apply ice melting when necessary to keep sidewalks free from ice.
No swearing
Lost and found items will be kept for 60 days then if not claimed they will be donated or thrown away.
Only 1 player at a time hitting live batting practice in each batting cage.
Rules for Glacier Outdoor Facilities (Koch and Glacier Fields)
All gates, sheds, storage facilities, and doors must be locked when exiting the fields. Even if someone is practicing after your team, it is still your responsibility to follow through in making sure that whoever is after you are reminded to lock up.
No team can practice or play a game at any of these fields without prescheduling it with Glaciers management. Please inform Glaciers management if your practice or game has been cancelled.
All teams must pick up all their trash before exiting the fields.
Teams cannot use fields when there is inclement weather or when they walk on the dirt infield and leave large indentations. If you are doing Batting practice outside of the batting cages, please put a rubber mat where the coach is pitching on the grass in the field.
Teams must rake their infield after they have used it. Proper raking would mean if it rained that night, there are no low spots or crevices on the dirt infield that would cause ponding.
Parent Agreement
By signing this document, I acknowledge that if any injury or property damage occurs during my participation in this activity, a court of law may determine that I have waived the right to pursue legal action against the Released Parties on the grounds of negligence. I acknowledge that I have been provided with adequate time to review this entire document and, if desired, to consult with legal counsel prior to signing. Furthermore, I understand that participation in this activity may not be offered to me should I choose not to sign this release. I agree that being afforded the opportunity to participate at the stated cost in exchange for executing this release constitutes a reasonable arrangement.
