GENERAL POLICIES: Dancer(s) are required to wear the proper class attire and hair must be tied back securely for each class. No jewelry is to be worn in any classes. (Please refer to Positive Dance Experience Dress Code for suitable attire in each class). Dancer(s) and Fitness Members should arrive no more than 5 minutes prior to the start of their class. No gum or cell phones are allowed in any classes.

MEDICAL EMERGENCIES: In the event of an emergency, I authorize Positive Dance Experience to use reasonable discretion, in rendering first aid and/or arranging for emergency medical care (including hospitalization), at my expense. In the event of an accident or injury during this program or any other organized event that results from this program, I will not hold the employees, contractors, and/or Positive Dance Experience responsible.

 PAYMENT POLICY: I understand that payments for classes must be made through the Parent Portal or paid in cash in person or by e-transfer. All physical cash must be given directly to Chelsea Robinson or the administrator.

ASSUMPTION OF RISK: I acknowledge that attending Dance and Fitness Classes are a physical activity that involves jumping, kicking, rotation, and coordinated body movements. I agree that myself/my dancer(s) participation in Positive Dance Experience involves inherent physical risks and I agree to assume the full risk of an bodily injuries (including death), damages, or loss which I/my dancer(s) may sustain as a result of any activities arising out of, connecting with, or in any way associated with my/my dancer(s) participation in Positive Dance Experience Dance and Fitness activities. I certify that I/my dancer(s) present level of physical condition is consistent with the demands of active participation in Dance and Fitness.

RELEASE OF INJURY: In the event of a possible injury, Positive Dance Experience employees/contractors may come in contact with the you/the dancer(s) to prevent and/or treat an injury. By participating in in-studio dance classes or fitness classes you are agreeing to waive any and all liability to Positive Dance Experience and its employees/contractors.

MODEL RELEASE AND AUTHORITY TO IDENTIFY: I acknowledge that myself/my dancer(s) or myself may be photographed or recorded for marketing or public relations purposes. I hereby grant Positive Dance Experience permission to use photographs or video of my dancer(s) or myself as elements of a photographic piece or to put the photographs to any legitimate uses that may deem proper such as in advertisements, social media, or by other means. Please note that you will need to agree to this term to complete your registration with us. If you are not comfortable with your dancer being photographed, please let us know and we will make a note on your file not to publish photos of your dancer.

PERSONAL PROPERTY: Be sure to label your belongings. Positive Dance Experience provides clients with cubbies during the class for safety of personal items, however, Positive Dance Experience is not held responsible for any lost or stolen items. We recommend that clients do not bring money or valuables to Positive Dance Experience. Any misplaced items will be placed in the lost and found bin at the end of each night, and donated at the end of the season.
