Expanded Observation Clinic Application

As an observer of the Canadian College of Homeopathic Medicine (CCHM) Observation clinic, I hereby undertake to strictly comply with the following conditions concerning the following materials:

- All Video Files via Google Classroom or any hosting provider.

- All Audio Files via Google classroom or any hosting provider.

- All photographs or Image Files on any online hosting provider.

1.Observer must retain in his/her possession and control all copies provided and not release them to anyone else.

2.No one is to have unsupervised possession or control of such materials for any purpose.

3. No one is permitted to view such materials except the Observers and persons acting as volunteers of the clinic who are subject to the same trust conditions (ie, staff or faculty member of CCHM or such other parties as may be authorized by CCHM to have access to these materials.)

4.These materials are not to be copied by anyone for any purpose.

5. These materials are to be used for the purpose of furthering the observers course of study while registered for the CCHM Observation clinic.

6. If the observer is unable to comply with these conditions, the observer shall first obtain the consent of CCHM, in writing, to deviate from these conditions.

7. This undertaking is binding upon the observer named above, and his or her heirs, successors, trustees, attorneys and assigns
