By submitting this form, you agree to the following:

Minimum booking of 1 hour per massage therapist.
Maximum booking of 6 consecutive hours per massage therapist. For bookings over 4 hours, a 30-minute unpaid break is required and not included in billing.
Paid parking costs, if required, will be charged to the client.
Insurance receipts are issued only for services of 30 minutes or more in employee self-pay bookings.
Cancellation or rescheduling must be made at least 7 business days in advance to avoid charges.
Company is responsible for providing a suitable space for massage services.
