Headspa Treatment Consent & Liability Waiver

I voluntarily consent to receive headspa services, including but not limited to scalp treatments, oils, masks, massage, and water-based services.

I confirm that I have fully disclosed any medical conditions, scalp disorders, allergies, sensitivities, or medications that may affect my service.

I understand that failure to disclose relevant information may increase the risk of adverse reactions, and I accept full responsibility for any outcomes resulting from nondisclosure.

I understand that all services are provided for relaxation and cosmetic purposes only and are not intended to diagnose, treat, or cure any medical condition.

I acknowledge that individual results may vary and no guarantees have been made regarding outcomes.

I understand there is a risk of irritation, allergic reaction, or sensitivity, and I voluntarily assume all such risks.

I release, waive, and hold harmless Mane Treatment Headspa & Salon, its owners, employees, and contractors from any and all liability, claims, damages, or causes of action arising from the services provided, except in cases of gross negligence.

I authorize the service provider to discontinue service if any unsafe or unsuitable condition is identified.

I confirm I am at least 18 years of age or have a parent/guardian consent.

Cancellation, No-Show & Late Policy Agreement

I understand that a minimum of 24-hour notice is required to cancel or reschedule my appointment.

I agree that late cancellations (less than 24 hours) may result in a cancellation fee up to 50–100% of the scheduled service.

I understand that failure to show up for my appointment (no-show) may result in being charged the full service amount.

I understand that future appointments may require a non-refundable deposit after a late cancellation or no-show.

I understand that arriving late may result in a shortened service or cancellation, and I may still be charged the full service amount.

I agree that all deposits and fees are non-refundable unless otherwise stated.

I grant permission for photos and/or videos taken during my service to be used for marketing, social media, and promotional purposes.
