I authorize Mary Anne Seeley, Neils Topham, any Quickstep LLC dance instructor or substitute dance instructor to secure the services of a physician or hospital and agree to incur the expenses of such for necessary services in the event of accident or illness while participating in any event or practice at Quickstep LLC

I understand that there are physical risks associated with dancing and I hold blameless and harmless, Quickstep LLC i.e. Topham Management LLC and its respective employees and I thereby assume all risk and responsibility for any injuries, illnesses or damages resulting from participation in the activities related directly or indirectly to Quickstep LLC. I assume responsibility for any and all respective medical costs incurred from such participation (i.e. practices, performances, travel, etc.). Students at Quickstep are dancing at their own risk.
