I ______________, hereby give my consent for my child, ______________, to participate in the Rise and Reach Project (the "Organization"). I understand that my child will engage in various activities, programs, and events organized by the Organization. I acknowledge and understand the nature of the activities that my child will participate in, including but not limited to mentorship, recreational events, fundraising initiatives, and other activities determined by the Organization. I am aware that my child may be interacting with mentors, volunteers, and other participants during these activities. I certify that my child is physically, mentally, and emotionally capable of participating in the Organization's activities and that my child has no medical or other conditions that would prevent or limit their participation. I understand that it is my responsibility to inform the Organization of any changes in my child's health or medical condition that may affect their participation in the activities. I acknowledge that participation in the Organization's activities involves inherent risks, including but not limited to the risk of injury, accidents, or illness. I understand that the Organization takes reasonable measures to ensure the safety and well-being of its participants, but I recognize that accidents or incidents may still occur despite these efforts. I authorize the Organization and its officers, directors, employees, and agents to take any reasonable measures necessary to protect the health, safety, and welfare of my child during their participation in the Organization's activities. This may include obtaining emergency medical treatment for my child, as deemed necessary by the Organization, and I agree to be responsible for any medical expenses incurred on behalf of my child. I release and hold harmless the Organization, its officers, directors, employees, and agents from any and all liability arising from or related to my child's participation in the Organization's activities, including but not limited to any injury, loss, or damage incurred by my child during their participation. I have read and understand the nature of the Organization's activities and hereby give my consent for my child to participate in the Rise and Reach Project.
