Elite Health Club Membership Agreement

This Membership Agreement (“Agreement”) is entered into between the undersigned client (“Client”) and Julie E Health. (“Provider”), effective as of the date signed below.

1. Program Commitment

Client understands and agrees that enrollment in the Elite Health Club requires a minimum commitment of six (6) months from the date of the first session under this membership.

  • Client agrees to schedule at least one (1) session per month with Julie E or her designated practitioner during this period.

  • This is a non-cancellable, non-refundable membership. No early termination is permitted.

  • After the initial 6-month period, Client may cancel the membership with 30 days’ written notice.

This commitment is designed to deliver long-term, transformational results through consistency and accountability.

2. Membership Benefits

During the term of this Agreement, Client will receive:

  • Preferred pricing on all sessions (below standard $350 rate)

  • Access to lifetime wellness resources and tools

  • Exclusive invitations to member-only education and promotions

  • Priority scheduling with Julie E and team

  • Curated recommendations for supplements, protocols, and tools

  • Personalized support and community engagement

Note: Benefits are subject to change as new offerings become available.

3. Session Scheduling & Attendance

  • All sessions are by appointment and must be scheduled via the Provider’s online booking system.

  • Missed sessions do not roll over to subsequent months unless otherwise agreed in writing.

  • Sessions begin and end on time. A 7-minute grace period is allowed. Beyond that, time may be pro-rated at your session rate.

4. Cancellations & Rescheduling

We value your time and appreciate the same respect. Please review our cancellation policy:

  • Cancellations or rescheduling must be made at least 24 hours prior to your session.

  • Sessions canceled within 24 hours or no-shows will be charged in full, even for EHC members.

  • Multiple reminders are sent to help you remember and honor your appointment.

  • Missed sessions affect both your progress and others — we often have a waitlist for high-demand time slots.

5. Payment Terms

  • Membership is billed at $[insert amount] per month, for six (6) months, processed automatically to the card on file.

  • Client authorizes Julie E Health to charge the card on file for all monthly membership fees, including late cancellations.

6. Scope of Services & Disclaimers

  • Julie E and her staff are not licensed medical doctors, and services provided are educational and holistic in nature.

  • Any recommendations, assessments, or protocols are not intended to diagnose, treat, or cure any disease.

  • Client assumes full responsibility for their own decisions related to diet, supplements, and lifestyle.

7. Confidentiality

All sessions are confidential. Your personal information and discussions are never shared. Julie may reference anonymized examples in educational settings, but your identity will never be disclosed.

8. Client Representation

By signing below, the Client affirms the following:

  • They are over the age of eighteen (18).

  • They are not acting on behalf of any federal, state, or local agency.

  • They are entering into this Agreement voluntarily and with full understanding of its terms.

9. Entire Agreement

This document constitutes the full agreement between Client and Provider. It supersedes all prior communications. Amendments must be in writing and signed by both parties.

I have read the above policies and I agree to all of the terms and conditions. I authorize Julie E Health permission to charge my credit card that you have on file if I have a late cancellation or a no-show for an appointment.
