ASSUMPTION OF RISK & LIABILITY WAIVER

By signing below, I understand that participation in rhythmic gymnastics and intensive athletic training involves inherent physical risks, including but not limited to sprains, strains, fractures, and other injuries. I voluntarily assume all risks associated with participation.

I release and hold harmless the school, its owners, coaches, employees, and representatives from any liability, claims, demands, or causes of action arising from participation in the Summer Intensive Camp.

MEDICAL TREATMENT AUTHORIZATION

By signing below, , I acknowledge that in the event of an emergency, I authorize the staff to obtain medical treatment for my child if I cannot be reached immediately.

PHOTO & VIDEO RELEASE

By signing below, I give permission for my child to be photographed and/or recorded for promotional and marketing purposes, including use on the website and social media.

PAYMENT & ATTENDANCE POLICY

By signing below, I understand that registration secures my child’s place in camp. Selected weeks must be confirmed in advance and payment deadlines must be followed. Spaces are limited and non-refundable unless otherwise agreed in writing.
