This form collects the information needed to apply for enrollment in the Red Cedar Ag Harvest Guard program. Completion of this form does not constitute final enrollment.

After submission, a formal contract will be sent to the participant’s email via DocuSign. Enrollment is not finalized until the contract is reviewed and signed. All information provided must be accurate and complete. Red Cedar Ag reserves the right to review submissions and request additional information prior to issuing a contract.

This form must be completed by January 31, 2026. Submissions received after this date may not be accepted.

By submitting this form, the customer indicates their intent to commit to the crop allocation volume(s) listed, subject to final contract approval. Submission of this form acknowledges acceptance of these terms.
