Alex's Dog Training Service Contract

Liability Waiver and Policies

1. General: The undersigned (“you The undersigned (“you” or “Participant”) acknowledges that he/she is voluntarily participating in dog training provided by Alex’s Dog Training with knowledge of the potential dangers.

* You acknowledge that you are participating in an online and in person program with Alex’s Dog Training. This program may include but is not limited to pre recorded video lessons, in person lessons, assignments, private sessions done through video call, group video calls, and individual messaging. Alex’s Dog Training will provide you with training information and assist through the aforementioned methods as you implement said training information. You acknowledge that you will attend the program fully prepared to accomplish homework assignments, and that by beginning this program you take full responsibility for your dog, as well as your dog’s behavior and training.

* Participant will schedule program start date based on program availability. The program will be paid for in 6 payments of $250, for a total of $1500. First payment is due by the start date of the program.

You acknowledge that you commit to purchasing this program as a whole. Monthly payment options are offered as a courtesy to you, the Participant, but do not negate your financial commitment to pay the total of the amount of the program. Exiting the program before the 6 month period is over does not negate your financial commitment for the entire program. No refunds will be made, and any remaining payments will be charged in full. Alex’s Dog Training reserves the right to charge the full amount of the program at any time.

* Private training sessions as part of the program will be scheduled once the program has started, and may or may not be at regular intervals throughout the program. Once scheduled a training session cannot be canceled by the Participant (but may be rescheduled as outlined below). If the participant does not attend a training session they will not be refunded in any amount for said session. Alex’s Dog Training reserves the right to reschedule at any time and for any reason. If Alex’s Dog Training reschedules a training session we will work with the Participant to find another date for the training session.

*You will receive 2 in person and 2 virtual sessions (alternating weeks) per month for the first two months of the program. You will receive one phone session per month for the remaining 4 months of the program. You will receive an additional 4 in person training sessions to be used as needed throughout the six month program.

* In the event that the Participant desires to reschedule a training session, notice of at least 24 hours must be given. If there are no available times to reschedule to, the client can either keep their initial time slot or lose the session with no reimbursement, financial or otherwise. By accepting these terms and conditions the Participant acknowledges that no refunds will be made for any reason. This is a commitment to completing the program as a whole.

All services offered in this program must be completed within the 6 month time frame. Services from this program are not available once the 6 month program has ended, even if client did not pursue full completion of services within the time frame of the program.

You acknowledge that any and all photos taken by Alex’s Dog Training or sent to Alex’s Dog Training by the participant in any form, i.e. electronic or other, are property of Alex’s Dog Training and that Alex’s Dog Training may use and publish the same in print and/or electronically. I agree that Alex’s Dog Training may use such photographs of me and/or my pet with or without my name and for any lawful purpose, including, for example, such purposes as publicity, illustration, advertising, and Web content.

* You acknowledge that this program is intended for the use of responsible adults, and any participation in or use of this program by minors or any persons that are not fully of sound mind is at your own risk. You acknowledge that Alex’s Dog Training cannot be held responsible for the risks associated with minors or unsound persons participating in this program.

* You recognize that the role you play in your dog’s learning process is integral to achieving desired results. You acknowledge and agree that there is no guarantee that your dog will achieve any desired level of training, despite the best efforts of Alex’s Dog Training. You further understand that the recommendation of any other product or service by Alex’s Dog Training is not a guarantee of satisfaction with that product or service. I fully understand that the Alex’s Dog Training has not represented, promised, guaranteed, or warranted that said dog can be trained, that said dog will not be dangerous or vicious in the future, that said dog will not exhibit other behavioral problems, or that the training will last for any particular amount of time.

* Alex's Dog Training can be contacted via phone, email, or text throughout the duration of the program. Despite best efforts to respond within 24 hours, there is no guarantee of response times. Any communication received on Sundays will be responded to the following week.

* You acknowledge that scheduling may be affected by weather or other unforeseen circumstances. In this situation best efforts will be made to find a suitable day and time to reschedule to.

* Alex’s Dog Training is not qualified to advise on veterinary and canine health practices including but not limited to medication and diet, human/owner physical or mental health practices, or anything outside the topic and aim of this dog training program. Anything discussed outside of dog training is strictly the personal opinion of the members, employees, trainers, volunteers, etc. of Alex’s Dog Training and Alex’s Dog Training or it’s members, employees, trainers, volunteers, etc. are not responsible for said opinions.
