By signing below, I, the undersigned, confirm that I understand and agree to the terms outlined in this consent form regarding my permanent jewelry services. I acknowledge that the jewelry I am receiving is welded on and is intended to be permanent.

Waiver of Liability:

I understand and acknowledge that although permanent jewelry is designed to be durable, it is not impervious to damage, wear, or other external factors. As such, I waive any and all claims against Backstory Ink & Link, LLC, its employees, affiliates, and contractors for any injuries, damages, or issues arising from the application, wear, or removal of my permanent jewelry.

I further understand that while every precaution is taken to ensure the safety and effectiveness of the procedure, complications can arise in some cases, and I take full responsibility for my own health and safety post-procedure.

No Refund Policy:

I understand and agree that all services provided are final, and no refunds will be given for any permanent jewelry treatments or products purchased.

In the event that my permanent jewelry needs to be rewelded for any reason (e.g., chain breakage or disconnection), I understand that the rewelding will be provided at no additional charge as long as the service falls within a reasonable timeframe and under normal wear conditions.

Consent to Treatment:

I give my consent for the technician to perform the permanent jewelry service as described to me, including the application of the jewelry, welding process, and any necessary adjustments during the procedure. I have been informed of the process, aftercare instructions, and potential risks associated with permanent jewelry.

Aftercare Instructions:

I acknowledge that I have received and understand the aftercare instructions for my permanent jewelry. I will follow these instructions to ensure proper healing and to maintain the integrity of the jewelry.
