Medical and Participation Waiver

I confirm that I am physically able to participate in trail running and circuit training. I understand these activities involve risks, including falls, injury, exhaustion, and illness. I voluntarily accept these risks and authorize the event team to arrange emergency medical assistance when reasonably necessary. I release Coach Sandi and the event team from claims arising from the ordinary risks of participation, except where prohibited by law.

Media Consent — Optional

I authorize Coach Sandi and the event team to photograph or record me during the workshop and use my image, voice, or likeness for event documentation and promotional materials without compensation. I understand that I may withdraw consent for future use by contacting [email protected], although this will not affect materials already lawfully published.

Photos and videos of identifiable people are personal information under Philippine privacy rules; consent should be informed, specific, recorded, and capable of withdrawal.
