Parent/Guardian Agreement

By submitting this form, I, the undersigned parent or legal guardian, give permission for my child(ren) to participate in the Kingdom Business Network's Young Entrepreneurs Club Workshop (“the Workshop”). I understand and agree to the following terms:

1. Assumption of Risk & Release of Liability

I understand that participation in the Workshop may involve physical activities, hands‑on projects, and group interaction. I acknowledge that accidents, injuries, or unforeseen incidents may occur.

To the fullest extent permitted by law, I release, waive, and hold harmless Kingdom Business Network, its directors, officers, volunteers, employees, agents, and partners from any and all liability, claims, demands, or causes of action arising from or related to my child’s participation in the Workshop, including but not limited to:

- Accidental injury

- Illness

- Property damage

- Participation in activities on or off site

This release applies whether the harm is caused by negligence or otherwise, except in cases of gross negligence or intentional misconduct.

2. Medical Information, Allergies & Special Needs

I agree to provide complete and accurate information regarding my child’s:

- Allergies (food, environmental, medication, etc.)

- Medical conditions

- Required medications

- Physical, behavioral, or learning needs

I understand that Kingdom Business Network cannot guarantee an allergen‑free environment and that I am responsible for communicating any accommodations my child(ren) require(s).

3. Emergency Medical Authorization

In the event of an emergency where I cannot be reached, I authorize Kingdom Business Network and its representatives to:

- Seek emergency medical care for my child

- Administer basic first aid

- Contact emergency services if necessary

I agree to be responsible for any medical expenses incurred.

4. Behavior & Participation Expectations

I understand that my child is expected to:

- Follow instructions from staff and volunteers

- Treat others with respect

- Participate in activities in a safe and responsible manner

If my child’s behavior creates a safety concern or disrupts the Workshop, I understand they may be dismissed from the event.

5. Personal Belongings

I understand that Kingdom Business Network is not responsible for lost, stolen, or damaged personal items brought to the Workshop.

6. Media Release

I grant permission for Kingdom Business Network to photograph or record my child during the Workshop for use in nonprofit promotional materials, social media, and documentation. I understand that no compensation will be provided.

7. Consent & Acknowledgment

By signing/submitting this waiver, I acknowledge that:

- I have read and understand all terms

- I am the parent or legal guardian of the child listed

- I voluntarily agree to these conditions as a requirement for participation
