Your child wants to participate in the Built Differently Athlete DNA Scan and DNA Index assessments. These tools are designed to help young athletes identify behavioral patterns that impact their performance on and off the field.
Before your child can participate, you must complete and sign the permission slip below. No athlete under 18 will be assessed without verified parental consent on file.

The Built Differently DNA Scan is a brief introductory assessment designed to identify the key behavioral patterns that shape how your child approaches competition, preparation, and performance. It serves as the entry point into the Built Differently performance system and gives your athlete their first clear look at how they are wired.
The Built Differently DNA Index is a comprehensive behavioral performance assessment that goes deeper - measuring the specific gaps between what your athlete knows they should do and what they actually execute when it matters most. The results produce a personalized performance profile that coaches and athletes use to target the exact areas holding performance back.
Together, these two tools give your child and their coaching team a data-driven foundation for real, measurable development - on the field and beyond it.
