We are so excited to expand our foster base so we can continue saving dogs and cats from death row! We take pride in training and educating all of our foster families to ensure everyone is successful and happy. Please answer questions to the best of your ability. Foster Agreement and Waiver of Liability
Fosters provide temporary homes to animals and help rescues save more animals from euthanasia. A foster home may have an animal for a few weeks to a few months. There is no guarantee of exactly how long the animal will be in foster care. You must read this contract thoroughly and understand exactly what is required of you as a foster. By Signing this agreement I the foster agree to accept the following conditions and restrictions in order to be eligible as a foster home for Operation Freedom Ride Inc. (OFR). In order to foster for OFR, adult members of the household (i.e. partners, spouses, children 18 and over) must agree to the conditions found within this contract and indicate their agreement by signing below.
DAILY CARE: I understand and agree to provide the foster animals with food, water, exercise, socialization, obedience training. I agree to provide the care and attention necessary to ensure the animal adapts to their new temporary home. No animal should be kept outdoors. Obedience training and crate training may be just a few of the tools necessary to help the foster adjust.
REIMBURSEMENT: OFR will cover all costs associated with the foster such as vetting and food. Any expenses must be pre approved by your OFR coordinator in order to receive reimbursement. I understand in the rare case my personal animal, or human family members get sick from the foster(s) or while the foster animal is living with me, that I am responsible for the cost of vetting my animal and persons in the home and OFR does not have to reimburse me. I understand and agree that OFR ONLY covers vetting costs that are pre approved and at one of the veterinary offices in OFR's network.
SPAY/NEUTER/VETTING: I understand and agree that I may be required to bring the foster animal to the vet for altering or any other needed vetting. I agree to be made available to drive and attend the visit with the foster at a time that works for myself and OFR. I agree that in the event a vet apt is scheduled for my foster and I am unable to make the appointment I must give 48 hrs notice or I will be billed the no show/cancelation fee.
I understand that OFR does not guarantee the health and temperament of any animal.
All animals from out of state will arrive with a minimum of 1 round of vaccines, dewrmer and a health certificate from a vet prior to transport. This does NOT guarantee the health of the animal against disease. We always insist you keep fosters separate from other animals for a min of 5 days for health and mental decompression.
I understand and agree that there are some diseases that can be spread to humans and other animals and understand that OFR does not guarantee the animal is free of these diseases upon entering the foster home and OFR is not responsible if said disease is spread in the foster home.
I agree to report all health concerns to an OFR representative immediately and will be made available to bring the foster to the vet. While OFR will attempt to see that the foster dog is compatible with the foster home and healthy, OFR does not guarantee the foster animal's temperament or behavior in any way.
OWNERSHIP: I understand I may not give, lease, sell or otherwise transfer custody of the rescue animal to anyone while it is under the foster homes care. OFR will retain ownership of all foster animals and will make final decisions (with the input of the foster home) regarding adoption or euthanization. I understand that OFR may, as it deems necessary, make visits to the foster home to check on the foster animal status. Should OFR decide that the foster animal requires any behavioral training or medical attention, the foster home agrees to abide by OFR’s decision(s). I understand and agree that while the foster is in my home and under my care that I am responsible and liable for the animals actions.
BITES: I understand and agree that OFR and or any of its volunteers or employees will not be held responsible in the rare case that a foster bites a person or animal. I agree that while I have the foster(s) in my care that I am responsible for the actions of the animal not OFR. I agree that I will report to OFR any bite (whether inflicted on a human or another animal) within 1 hours of the incident. The foster home will abide by the decision of OFR as to the disposition of the rescue animal should a bite occur.
I understand and agree that OFR does in the rare event an animal should become severely ill or aggressive, use humane euthanizia as an option. End of life is never easy to handle, if you are fostering an animal that has become ill or aggressive please let you're foster coordinator know if you would like to be present or updated on the decision of the animal.
OTHER: I understand that I will notify OFR immediately of any upcoming change of address or telephone number. I agree to provide a copy of the lease (if the property where the foster animal house is rented) stating that the said I have permission from the landlord/leasing agent to house a foster animal of that said type if asked. I agree that in the case I am unable to continue fostering the animal for the agreed upon time then I must give OFR a minimum of three days to find alternate housing for the animal.
ADOPTIONS: I understand and agree and will be made available to do meet and greets or events with potential adopters. I understand I will be working directly with potential adopters and am okay with being contacted with approved adopters. I will make myself available at a minimum on the weekends for meet and greets. I agree that in the case I want to adopt my foster animal I must let my OFR representative know immediately so I can be considered. If a meet and greet is already set up with another family then you do not have the option to adopt.
BY SIGNING I UNDERSTAND AND AGREE TO HOLD HARMLESS Operation Freedom Ride Inc. its employees, agents, members, directors, officers, or any affiliated entities thereof, from ANY LOSS, LIABILITY, DAMAGE, OR COST INCLUDING BODILY INJURY OR PROPERTY DAMAGE ARISING OUT OF OR RELATED TO THE FOSTERING OF AN ANIMAL FROM OPERATION FREEDOM RIDE INC. I AGREE I HAVE READ THIS FOSTERING AGREEMENT, RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
