Proof of Identification
According to Maryland laws and regulations, a Notary Public must obtain a brief description of the method of identification and the identification credential presented, if any, the Notary must record the date of issuance and expiration of any identification credential, and the identification number associated with the identification credential (State Gov't § 18–222)
Unless your notarization requires two forms of identification per the instructions given by the requesting organization it is sufficient to upload a U.S. driver's license OR U.S. state identification OR U.S. Passport OR NON-MILITARY identification with a photo and a signature.

Additional Proof of Identification
Only complete this section if you have uploaded an identification credential without an address.
According to Maryland statutes and regulations, the Notary Public may require an individual to provide additional information or identification credentials necessary to assure the Notary Public of the identity of the individual (State Gov't §18–206). If your residential address is absent from your identification document, please provide a document that displays your current residence (State Gov’t §18–219).

A Notary Public cannot notarize copies of vital records (i.e. birth, death, marriage certificates, and divorce decrees) and naturalization documents.
