Privacy Policy
Please provide accurate medical information to help us ensure your child's safety during tennis activities. All information is kept confidential and shared only with coaching staff on a need-to-know basis.
MEDICAL TREATMENT AUTHORIZATION AND CONSENT
I, the undersigned parent or legal guardian of the minor participant named in this registration ("Participant"), hereby authorize SRQ Tennis, LLC, its owner Michael Boothman, coaches, instructors, and staff ("Authorized Personnel") to act on my behalf in matters concerning the health and safety of Participant during tennis activities.
EMERGENCY MEDICAL TREATMENT
In the event of a medical emergency, injury, or illness involving Participant, I authorize Authorized Personnel to:
- Administer basic first aid
- Contact emergency medical services (911)
- Arrange for transportation to the nearest appropriate medical facility
- Consent to emergency medical treatment, including but not limited to diagnostic testing, medical or surgical procedures, and administration of medications deemed necessary by licensed medical professionals when I cannot be reached in a timely manner
EMERGENCY CONTACT EFFORTS
I understand that Authorized Personnel will make reasonable efforts to contact me or my designated emergency contact before seeking medical treatment. However, if I cannot be reached and a delay in treatment could be harmful to Participant, I authorize Authorized Personnel to consent to treatment on my behalf.
FINANCIAL RESPONSIBILITY
I understand and agree that I am financially responsible for any and all costs associated with medical treatment, ambulance transportation, hospital stays, and any other related expenses incurred on behalf of Participant.
MEDICAL INFORMATION DISCLOSURE
I authorize Authorized Personnel to disclose the medical information provided in this registration to emergency medical personnel, hospital staff, or other healthcare providers as necessary for the treatment of Participant.
I, the undersigned parent or legal guardian of the minor participant named in this registration ("Participant"), understand that SRQ Tennis, LLC may capture photographs, videos, and audio recordings during tennis lessons, camps, clinics, and related activities.
GRANT OF PERMISSION
I hereby grant SRQ Tennis, LLC, its owner Michael Boothman, and authorized representatives the irrevocable right and permission to:
- Photograph and/or video record Participant during activities
- Use, reproduce, edit, publish, and distribute such photographs and videos in any media format, including but not limited to:
Website (srq.tennis)
- Social media platforms (Instagram, Facebook, etc.)
- Email newsletters and marketing materials
- Printed promotional materials
- Press releases and media coverage
TERMS
- No compensation is expected or will be provided for the use of Participant's likeness
- Participant's name will not be published alongside images without separate written consent
- SRQ Tennis, LLC will use images in a respectful and appropriate manner consistent with our brand values
- This release remains in effect unless revoked in writing
LIABILITY WAIVER, RELEASE, AND ASSUMPTION OF RISK
I, the undersigned parent or legal guardian of the minor participant named in this registration ("Participant"), hereby acknowledge and agree to the following:
ASSUMPTION OF RISK
I understand that participation in tennis instruction, camps, clinics, and related activities ("Activities") offered by SRQ Tennis, LLC involves inherent risks, including but not limited to:
- Sprains, strains, fractures, and other musculoskeletal injuries
- Cuts, bruises, and abrasions
- Heat-related illness including heat exhaustion and heat stroke
- Dehydration
- Impact from tennis balls, racquets, or other equipment
- Collisions with other participants, nets, fences, or court surfaces
- Injuries resulting from slips, trips, and falls
- Exposure to outdoor elements including sun, wind, rain, and insects
- Aggravation of pre-existing conditions
I voluntarily assume all risks associated with Participant's involvement in these Activities, whether known or unknown, foreseeable or unforeseeable.
RELEASE AND WAIVER OF LIABILITY
In consideration of Participant being permitted to participate in Activities, I, on behalf of myself, Participant, and our respective heirs, assigns, and personal representatives, hereby release, waive, discharge, and covenant not to sue SRQ Tennis, LLC, its owner Michael Boothman, coaches, instructors, employees, volunteers, agents, and any facility owners or operators where Activities take place (collectively, "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by Participant while participating in Activities, or while on or upon the premises where Activities are conducted.
INDEMNIFICATION
I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, suits, losses, damages, costs, and expenses (including reasonable attorney's fees) arising from or related to Participant's participation in Activities or any breach of this Agreement.
ACKNOWLEDGMENT
I have read this Liability Waiver, Release, and Assumption of Risk in its entirety. I fully understand its terms and understand that I am giving up substantial legal rights, including the right to sue. I acknowledge that I am signing this Agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
By checking this box, I acknowledge that I am the parent or legal guardian of the Participant and that I agree to all terms of this Liability Waiver, Release, and Assumption of Risk on behalf of myself and the Participant.
SRQ Tennis is committed to providing a positive, safe, and respectful environment for all participants. By enrolling in our programs, I agree that the Participant named in this registration will adhere to the following standards of behavior:
RESPECT
- Treat coaches, staff, fellow participants, and facility users with courtesy and respect
- Use appropriate language at all times; profanity, slurs, or derogatory remarks are prohibited
- Respect equipment, facilities, and the property of others
SPORTSMANSHIP
- Demonstrate good sportsmanship during all activities, drills, and match play
- Accept coaching instruction and feedback with a positive attitude
- Win with humility and lose with grace; no racquet throwing, ball abuse, or verbal outbursts
EFFORT & PARTICIPATION
- Arrive on time, properly dressed, and ready to participate
- Give full effort during all activities
- Stay hydrated and communicate any physical discomfort to coaches
SAFETY
- Follow all safety instructions provided by coaches and staff
- Report any unsafe conditions or behaviors to coaching staff immediately
- Keep the court area clear of personal belongings and hazards
CONSEQUENCES
I understand that failure to adhere to this Code of Conduct may result in:
- Verbal warning
- Temporary removal from activity for a cool-down period
- Parent/guardian notification
- Suspension or dismissal from the program without refund
SRQ Tennis reserves the right to dismiss any participant whose behavior is disruptive, unsafe, or inconsistent with our program values.
Due to Florida's weather conditions, outdoor tennis activities may occasionally be affected by rain, lightning, or excessive heat. Please review and acknowledge our weather policy:
RAIN & LIGHTNING
- If lightning is detected or a storm is imminent, all outdoor activities will be suspended immediately
- Participants will be directed to a safe location until conditions improve
- If conditions do not improve within a reasonable time, the session will be canceled
EXCESSIVE HEAT
- Coaches monitor heat index and adjust activities accordingly
- Additional water breaks will be provided during high-heat conditions
- Sessions may be shortened or canceled if heat index exceeds safe levels
CANCELLATION NOTIFICATION
- Cancellations will be communicated via text message and/or email as soon as possible
- Please check your phone/email before departing for scheduled sessions on questionable weather days
MAKEUP POLICY
- Weather-canceled sessions will be rescheduled as makeup sessions when possible
Labor Day week (Sep 7–10) is reserved as the built-in weather makeup week. Weather makeups run at no charge; if you attend that week and no makeup is owed, it's billed at $37.50 per session
No refunds or prorated credits for weather cancellations; a makeup class is offered when SRQ Tennis reschedules
PARTICIPANT ABSENCE
If you choose not to attend due to weather concerns but the session is held, this will be counted as an absence and no credit or makeup will be provided

GROUP PROGRAMS
Commitment
By enrolling in a group program, you are committing to a specific class day and time. Your spot is reserved exclusively for your child, and our staffing, court reservations, and class sizes are planned accordingly.
Payment
- Group programs are billed in 4 week cycles, not calendar months. The first cycle payment is due at registration and is what holds your spot
- Cycles 2 and beyond are due on the posted cycle dates for the season
- If payment is not received within 7 days of the cycle due date, the spot may be released to the waitlist
Cancellation
- A minimum of 30 days written notice is required to cancel enrollment
- No refunds will be issued for partial cycles
No Makeups or Class Switching
– There are no drop-in visits this fall; enrollment is by 4-week cycle only
– Mid-season joiners start at the next cycle date (no prorated partial cycles)
- Missed classes due to participant absence, illness, vacation, or scheduling conflicts will not be made up
- Switching between class days or times is not permitted; your enrollment is a commitment to your assigned class
- If you need to change your class day/time, you must cancel your current enrollment (with 30 days notice) and re-enroll in a different class if space is available
Cancellations by SRQ Tennis
If Coach Michael must cancel a scheduled class (weather or otherwise), a makeup class will be offered
CAMPS & CLINICS
- Full payment is required at the time of registration to secure your spot
- Cancellations made 14 or more days before the start date: Full refund minus a $25 administrative fee
- Cancellations made 7–13 days before the start date: 50% refund
- Cancellations made less than 7 days before the start date: No refund; credit toward future camp may be issued at the discretion of SRQ Tennis
- No refunds for partial attendance or early withdrawal once the camp has begun
PRIVATE LESSONS
- Cancellations made 48 or more hours in advance: Full refund
- Cancellations made 24 to 47 hours in advance: $75 cancellation fee
- Same day cancellations or no shows: Full lesson fee will be charged
WEATHER CANCELLATIONS
- Weather-related cancellations are handled per our Weather Policy
A makeup class is offered for sessions canceled by SRQ Tennis due to weather (see Weather Policy)
