I acknowledge and agree to the following terms:

1. Voluntary Participation: I am voluntarily participating in activities at The Node Chicago, including but not limited to sound baths, movement activities, and social gatherings.

2. Assumption of Risk: I understand that any physical activity carries inherent risks. I knowingly assume all risks associated with my participation, whether known or unknown.

3. Medical Condition: I affirm that I have no known medical conditions that would prevent my safe participation in the event activities.

4. Waiver and Release: I hereby release and forever discharge The Node Chicago, its owners, employees, contractors, and agents from any and all claims, liabilities, or causes of action arising from my participation.

5. Indemnification: I agree to indemnify and hold harmless The Node Chicago from any loss, liability, damage, or cost incurred related to my participation.

6. Binding Effect: This waiver is binding upon me and my heirs, executors, and assigns.

7. Photograph and/or record me during this event

8. Use, reuse, publish, and republish these images/recordings in any and all media

9. Use my likeness for promotional, commercial, advertising, and marketing purposes

10. Edit, alter, or composite these materials without restriction

11. Use these materials without compensation or additional approval
