Packet Use

The team representative completes the team registration once. A parent or legal guardian must review and electronically sign one Individual Player Packet for every participating player before that player takes the field. The online registration system should require completion of every mandatory field and retain a time-stamped copy of the signed record.

Submission Checklist

Proof of Team

REQUIRED BEFORE THE TEAM IS CLEARED TO PLAY. Upload a current Certificate of Insurance (COI) in PDF, JPG, or PNG format through the online registration portal. The certificate must remain valid through September 13, 2026 and satisfy the coverage and additional-insured requirements communicated by Coastline Select Baseball, the facility, or the permit holder.

Official Team Roster

Submit the final roster by the tournament deadline. Every listed player must have a signed Individual Player Packet on file. Use legal names that match eligibility documents.

Coach / Team Representative Certification

By signing below, the undersigned represents and certifies, on behalf of the team and organization, that:

□ The roster information and eligibility documents submitted are accurate and complete.

□ Each player has permission from a parent or legal guardian and a signed Individual Player Packet on file before participating.

□ The team will maintain required insurance and provide evidence of coverage upon request.

□ Coaches will follow tournament rules, facility rules, safety instructions, pitching limits, equipment standards, sportsmanship requirements, and decisions of tournament officials.

□ The team is responsible for supervising its players before, during, and after games and for promptly reporting injuries, unsafe conditions, or misconduct.

□ No coach will knowingly permit a player to participate while injured, ill, under concussion protocol, or otherwise medically restricted.

□ The team will not add an ineligible or unregistered player and understands that violations may result in removal, forfeiture, or other tournament discipline.

□ The signer has authority to register the team and bind the team/organization to the administrative tournament terms.

Team Financial and Event Terms

The team acknowledges that schedules may change because of weather, field conditions, safety, government/facility direction, or operational needs. Refunds, credits, forfeits, protests, gate fees, and cancellation procedures are governed by the published tournament rules and registration policy in effect when payment is made. This form does not replace those published terms.

Tournament Fee:

$300 per team (Early Bird)

After August 24th $400 per team (Regular Fee)
