NDIS PLAN

Your NDIS Number is the Reference # on your letter from the Agency and is also listed at the start of your NDIS Plan.
The Plan Start Date is listed on the first page of your NDIS Plan and also at the start of your funding breakdown.
This is the date that you want to start your agreement with Find Wellbeing Plan Management

This Service Agreement is for a participant in the National Disability Insurance Scheme, and is made between the participant or the participant's representative and the Plan Manager of Find Wellbeing Management Plan. NDIS Provider Number 4-KPQ53LA

Primary Contact Details

Representative The representative must be one the following:(a) A Plan Nominee as defined by the NDIA(b) A Correspondence Nominee as defined by the NDIA(c) A Child Representative as defined by the NDIA(d) A personauthorised to act for the participant under a current, valid and operative power of attorney(e) A person appointed as the guardian or manager of the participant by Court/Tribunal Order(If you would like to nominate a family member or friend to be your alternative contact please fill out the 'alternative contact' fields.)
