How to Choose Your Membership
Core Elevate ($149) Best for weight loss, vitamins, and TRT maintenance.
Choose Lifestyle VIP ($249). Best for optimization and longevity. Includes 20% savings on all à-la-carte compounded medications, aesthetic treatments such as Botox® and dermal fillers, as well as stem cell and exosome therapies.

Billing & Commitment
Membership payments are billed monthly on the date this form is submitted. Cancellation is available at any time after the initial 3-month commitment.
Medication Requests
Monthly automated reminders are sent for lab requisitions and medication refills. Submit the Med Request Form at least two weeks in advance to allow for review and shipping.

Overdue Payment Policy
• Membership payments are due as agreed in your membership contract and are processed through automatic billing.
• If your account becomes more than 45 days past due, your membership will be cancelled and all membership benefits will be suspended.
Reactivation Policy
• To reinstate your membership after cancellation, a $150 reactivation/start-up fee will apply.
• Reactivation requires payment of any outstanding balance plus the $150 reactivation fee before services can resume.
